Along with Krystal’s ENROLLMENT PACKET, the following documents are required in order to complete registration:
For all incoming kindergarten students that live in the Lime Street/Kingston attendance boundaries (must be 5 years old by December 2)
- Birth Certificate, a certified copy of birth record, a baptism certificate, passport, immigration certificate or a Bible record
- Immunization Record
- Physical (form given at Parent Meetings/completed ASAP)
- Oral Health Assessment (form given at Parent Meetings/completed by May)
- Proof of Residence
For all incoming kindergarten students that live in the Hesperia Unified School District attendance boundaries, but not within Lime Street/Kingston attendance boundaries (must be 5 years old by December 2)
- All of the above
- Intra-district transfer (blue form/given at Parent Meetings)
For all incoming kindergarten students that do NOT live in the Hesperia Unified School District attendance boundaries, (must be 5 years old by December 2)
- All of the above
- Inter-district transfer (must be picked-up at hometown’s District Office)
For all incoming first grade students that live within the Lime Street/Kingston attendance boundaries, and are currently enrolled in one of those schools
- Proof of Residence
For all incoming first grade students that live within the Lime Street/Kingston attendance boundaries, and are NOT currently enrolled in one of those schools
- Immunization Record
- Physical (form given at Parent Meetings/completed ASAP)
- Proof of Residence
For all incoming first grade students that live within the Hesperia Unified School District’s attendance boundaries, and are currently enrolled in a Hesperia Unified School District elementary school, other than Lime Street/Kingston

For all incoming first grade students that do NOT live within the Hesperia Unified School District’s attendance boundaries, and are currently enrolled in a Hesperia Unified School District elementary school, other than Lime Street/Kingston

NOTE--ACCEPTABLE PROOF OF RESIDENCE MUST BE ONE OF THE FOLLOWING:
- A recent postmarked envelope or
- Utility turn-on notice or
- Bill with current name/address or
- Escrow papers or
- Rental agreement
A driver's license or personal check are not acceptable as proof of residence
If you and your child are currently living with a family member or friend, we must have proof of that person's address, along with a signed letter, stating that you and your child reside with them.
